Preparing for Your Appointment

In an effort to help our providers and decrease wait times during your appointment, we request that you come prepared to your appointment by bringing the following information:

  • ID and insurance card, as well as a referral if needed. This will prevent any delays or cancellations of appointments. If you are unsure if you require a referral, please contact our office.
  • Names of referring doctors and associated health providers such as ophthalmologists, endocrinologists, primary care providers and cardiologists.
  • Your past medical and surgical history, including any eyelid or orbital surgeries and who they were performed by.
  • Your allergies and current medications.
  • Recent (within the past 6 months) laboratory results. This is especially important for patients being seen for thyroid conditions.
  • Recent imaging of the face/head/orbits- if you are being seen for an orbital condition such as fracture, mass or thyroid eye disease, please bring a disc of your imaging. If you are unable to bring a disc, please have the report printed out. We will also ask when and where the imaging was performed.
